The School District of Clayton's Health Services Program strives to advance the academic success, well-being and lifelong achievement of its students. Health Services facilitates positive student responses to normal development, promotes health and safety, intervenes with actual and potential health problems and provides case management services.

In collaboration with the District’s educational staff, family members and community resources, Health Services professionals in every District building work to build each student’s capacity for acquiring, understanding and implementing the health attitudes and behaviors necessary for learning.

Student Illness Guidelines

The School District of Clayton has established a list of student illness guidelines to help parents determine whether or not to send their child to school. Attendance is encouraged unless one of the following exists:

Fever: Students with a temperature of 100 degrees Fahrenheit or above should not attend school. Students must be fever-free for 24 hours without the aid of medication before they may return to school.
Conjunctivitis/Pinkeye: Students with red, watery eyes, with or without purulent drainage, must remain at home until 3 doses of antibiotic eye drops have been administered and eyes are clear of purulent discharge.
Impetigo: This condition is highly contagious and requires treatment. Unless under the care of a health care provider, the student may not attend school.
Head Lice: The school must be informed if your child has head lice. Any live lice or nits within ¼" of the scalp must be treated. Treatment is required before returning to school. The student must be "checked in" by the school nurse before returning to class.
Pinworms: This condition is easily transmitted and requires treatment before attending school.
Vomiting/Diarrhea: If a student has vomited or had diarrhea during the night or just before school, he/she should not attend. Students should remain at home until symptom-free and able to tolerate regular food.
Sore Throat: A sore throat may require medical attention to determine if a strep infection is present and an antibiotic needed. If a throat culture is done, student should remain at home until results are known. If culture is positive, student may return to school after 24 hours of antibiotics, if no fever is present.
Rash: A student with a rash may be excluded until the cause of the rash has been determined. A physician's note may be required for re-admission to school.
Ringworm of Scalp and Skin: Student will be excluded from school until effective treatment is started. This may require a physician's note.
Runny Nose: If drainage is profuse and not clear, student should remain at home until symptoms improve.
Cough: A cough following a cold is not necessarily contagious and may be prolonged. If the cough is croup or is productive with non-clear discharge, student should remain at home until symptoms improve.
Chicken Pox: Students are excluded until all lesions have been crusted and there are no new lesions, generally day 6 after onset of rash.

A School District of Clayton policy states that any student too ill to attend the morning session should remain at home for the entire day.

Please report any communicable disease such as strep throat, chickenpox, etc., to the school nurse for the protection of the entire building. If you have any questions about your child’s attendance at school, contact your school nurse.

Screening Schedule

Health Screenings

Vision and hearing screens are essential to identify students at risk for vision and hearing problems that can affect a student's learning. Hearing screens are provided by a specialist from Special School District. If a potential concern is detected, the school nurse will notify the parent/guardian to recommend followup.

Early Childhood Center

Vision Screening: All students aged 3-5 years.
Hearing Screening: All students aged 3-5 years.

Elementary Schools

Vision Screening: All students
Hearing Screening: K, 1, 3, 5, any new students to the district and any student being served by Special School District.

Middle School

Vision Screening: 7th graders, any new students to the district, and students served by Special School District.
Hearing Screening: 7th graders, any new students to the district, and students served by Special School District.

High School

Vision Screening: 9th grade, any new students to the district, and students served by Special School District.
Hearing Screening: 9th grade, any new students to the district, and students served by Special School District.

Annual Student Health Information Form

School District of Clayton Physical Form

It is recommended (but not required) that students submit a physical prior to entering kindergarten, third, sixth and ninth grade. It is also recommended that all NEW students to the District, as well as any student with a chronic health condition, submit a physical form prior to starting school. Students participating in Clayton-sponsored sports need to submit an annual physical that clears them to play sports. High school students must have the MSHSAA Physical Evaluation Form completed by a physician. You may also obtain one from your school nurse.

Medication Administration Form

The Medication Administration Form needs to completed by a parent/guardian and the student’s physician for over-the-counter medications (with the exception of medications included on the emergency health form). A prescription medication bottle will suffice as a physician order, therefore parent/guardian permission is only necessary in the case of a current prescription medication. Always bring medications in their original packaging/bottles.

Allergy and Asthma Forms

Allergy Action Plan

Students with a history of a severe allergic reaction (e.g., food or insect stings) and a diagnosis of a life-threatening allergy should have an Allergy Action Plan on file with the school nurse. This form is completed by the student’s physician annually and communicates the MD’s orders for medical intervention specific to the student in the event of an allergic reaction.

Asthma Action Plan

Students who have asthma and need to use an inhaler at school should have an Asthma Action Plan that provides instructions for treatment at school in the event of an asthma attack or exacerbation of symptoms. This form needs to be completed by the student's physician annually.
